When something goes wrong and an AI-assisted decision is questioned, the follow-up question isn't "who signed?" — that always has an answer. The real question is: "what did that person evaluate before signing?" Without a record of that, the signature protects no one — not the organization, and not the professional who approved it, who is now exposed to having signed off on something they can't prove they reviewed with judgment.

Signing without recording doesn't shift the risk to the AI. It concentrates it on the person who signed, without the evidence they'd need to defend that signature later. Traceability doesn't protect the system — it protects the professional.
